3/3/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Jahsim upon arrival to the home setting. Upon arrival to the home setting, Jahsim appeared to be in a positive mood as he was playing his game with his sister. Ms. Shabazz verbalized some behaviors that he has been displaying within the school setting that caused him to get silent lunch as a result, he has been aggressive in the home. QMHP-C informed Ms. Shabazz that those behaviors would be addressed. 2. Behavior: Jahsim displayed hyperactive behaviors as he was jumping off his brothers toy car to the floor at times within the session. Once he was redirected by QMHP-C he regained focused for the remainder of the session 1.Intervention: QMHP-C confronted Jahsim on his poor attitude and encouraged Jahsim to become active during session. QMHP-C conversed with Jahsim regarding his feelings about his negative reports from school. QMHP-C engaged Jahsim in a discussion to trace the source of his increased aggression in school. QMHP-C held the Jahsim accountable for his actions and prompted him to do the same. 2. Intervention: Jahsim was taught that until he recognizes his behavior as an issue, he will not make the necessary changes to improve it. QMHP-C educated Jahsim on strategies and methods he could utilize to improve his behavior. QMHP-C discussed and processed Jahsimâs feelings of needing to fit in and not wanting to be a victim. 1. Response: Jahsim was initially lethargic and passively listened to QMHP-C. Jahsim gradually became more involved in session. Jahsim initially denied any increased aggression in school. Jahsim stated that he did not like school but was unable to give any viable reason why. Jahsim listened actively and gave feedback regarding his lack of motivation and the consequences he would face if this behavior continued. Jahsim had difficulty independently expressing how he could avoid trouble in school. 2. Response: Jahsim agreed to methods that he was taught that would assist him in reducing trouble in school. Jahsim was able to reiterate methods. Jahsim stated that that sometimes he joins in with others to pick on people so âthey wont end up picking on meâ. Jahsim listened actively and gave feedback and was able to openly express how his actions in school were wrong.
